Case discussion: An 84-year-old swimmer with a history of multiple skin cancers presents with a concerning lesion on his back. What would you do next?

This week's case discussion, submitted by Dr David Stewart, features an 84-year-old male who is a keen swimmer and has had multiple skin cancers (both melanoma and NMSC) in the past, hence the multiple scars.
The patient usually sees a dermatologist but presents to "have a few barnacles frozen off", at which time this marked lesion is noted on his back.
