In this general dermatology case discussion, we look at a 53-year-old fitness instructor with severe irritant contact dermatitis affecting both hands.
Background
She washes her hands frequently throughout the day as part of her work and has a long history of atopic dermatitis. Her eczema has persisted for much of her adult life, with red, cracked, swollen and blistered hands significantly affecting her ability to work and her quality of life.
She has previously been treated with ultra-potent topical corticosteroids, wet cotton gloves, emollients and UVB phototherapy, with limited success.
Presentation
Her disease is graded 4/4 for severity on the IGA scale, while her DLQI is 19/30, indicating a significant impact on daily life.
