What do you think of this lesion on the helix rim?

This week, we revisit a case from Dr David Stewart, in which an 84-year-old male patient is referred by another GP for a new, tender lesion on the rim of the helix.

The patient was given antibiotics but the lesion has not improved. He has a past history of squamous cell carcinoma and lentigo maligna.
